Combivent (ipratropium bromide/albuterol) pharmacologic effect

Combivent (ipratropium bromide/albuterol) is available in the form of aerosol for inhalation. This is a combined preparation with marked bronchodilator effect. It is used for the prevention and long-term treatment of asthma, chronic obstructive bronchitis. 1 dose of aerosol contains ipratropium bromide and albuterol.

Ipratropium bromide is an anticholinergic agent. It can inhibit reflex bronchoconstriction and reduce the secretion of the glands of the respiratory tract. This agent effectively prevents the narrowing of the bronchi and eliminates bronchospasm induced by stimulation of the vagus nerve.

Albuterol is a beta-2 adrenergic agonist. It helps relax smooth muscles and prevent bronchospasm. When used at the recommended therapeutic doses, albuterol has no adverse effect on the cardiovascular system (it does not cause increase in blood pressure).

The combination of albuterol and ipratropium bromide is used to produce an enhanced bronchodilator effect.

Ipratropium bromide is quickly absorbed after inhalation. This agent does not penetrate the blood-brain barrier. Albuterol is rapidly and completely absorbed after inhalation. It penetrates through the BBB, producing a concentration equal to about 5% of the plasma concentration.

Guidance for children and pregnant women

Combivent (ipratropium bromide/albuterol) is not used in the II and III trimester of pregnancy and during breastfeeding (unless the expected benefit to the mother outweighs any possible risk to the fetus).

Combivent (ipratropium bromide/albuterol) interaction

Corticosteroids, anticholinergics and xanthine derivatives may enhance the effect of Combivent (ipratropium bromide/albuterol) and cause serious side effects.

MAO inhibitors and tricyclic antidepressants may enhance the effect of albuterol and lead to a sharp decrease in blood pressure.

Theophylline and other xanthines may increase the risk of tachyarrhythmia.

Diuretics and corticosteroids increase the hypokalemic effect of albuterol.

Anticholinergic drugs increase the risk of high intraocular pressure.

Combivent (ipratropium bromide/albuterol) overdose

Overdose manifestations are rare (due to the low systemic absorption following inhalation or ingestion). As a result, all the manifestations of overdose can be associated with albuterol.

Overdose symptoms include headache, nausea, vomiting, angina pectoris, hypertension, hypotension, hypokalemia, hyperkalemia, tachycardia, arrhythmia, chest pain, tremor, flushing, anxiety, hallucinations and dizziness.

Overdose treatment: symptomatic treatment is recommended.

Combivent (ipratropium bromide/albuterol) dosage

Use Combivent (ipratropium bromide/albuterol) 2 to 4 times a day. If necessary, patients may increase the multiplicity of dosing to 12 times per day (the maximum daily dose).

Combivent (ipratropium bromide/albuterol) contraindications

Do not use Combivent (ipratropium bromide/albuterol) in case of hypersensitivity to albuterol, ipratropium bromide, atropine or its derivatives, hypertrophic obstructive cardiomyopathy, tachyarrhythmia; pregnancy (I trimester). The drug is not recommended for children up to 12 years of age.

Precautions: angle-closure glaucoma, obstruction of the urinary tract (including benign prostatic hyperplasia), pheochromocytoma, hyperthyroidism, diabetes mellitus, cystic fibrosis, myocardial infarction, pregnancy (II-III trimesters), lactation.

Combivent (ipratropium bromide/albuterol) side effects

CNS: headache, dizziness, fatigue, tremor, paresthesia, insomnia, nervousness, incoordination, dysphoria, impaired memory, anxiety, depression. Cardiovascular system: palpitation, tachycardia, arrhythmia. Digestive tract: dry mouth, nausea, vomiting, diarrhea. Metabolism: hypokalemia, hyperglycemia. Respiratory system: cough, dysphonia, nasal congestion, paradoxical bronchospasm, dyspnea. Musculoskeletal system: tremor, myalgia, muscle cramps, muscle weakness, arthralgia. Genitourinary system: urinary retention, dysuria. Allergic reactions: angioedema, skin rash, laryngospasm, bronchospasm, pruritus, anaphylactic shock. Other reactions: mydriasis, angle-closure glaucoma, alopecia.

Combivent (ipratropium bromide/albuterol) cautions

Do not exceed the maximum daily dose to prevent overdose. Patients should be instructed on the correct use of the drug. Patients with asthma or COPD (moderate forms) should choose symptomatic treatment. It is necessary to analyze the further anti-inflammatory therapy to control airway inflammation in patients with steroid-dependent asthma and COPD.

Regular use of high doses of Combivent medication may worsen the disease. Patients should review the treatment plan to prevent life-threatening complication of the disease.

I’ve been taking this for more than 20 years and I never have had side effects. I would like to add that this works perfectly if you take this as your doctor prescribed to you. There is no necessity to raise the dose level I think. In my opinion for the better effects you can take other drugs with it. My doc suggested me to try it with Spiriva, and it worked perfectly without any problems. Don’t try it with Symbicort. There are a lot of side effects that you can get if you’ll try this. I used it for some stupid reasons and after some weeks after using it I started to observe that I started to have hair loss all over my body, my skin become dry and bad sensations of itchiness. I stopped taking this immediately. I feel now pretty well and happy about this medication. I really hope that it will help you too. The only important thing is to use it like your doc prescribed it to you, if you don’t want to have different side effects. I sincerely think that most of those that write here about side effects that they got from this or about another drug, it’s because they used it more that it was possible. So don’t commit this kind of errors and be careful about how much do you use the drug.

My condition is bronchi muscle spasm resulting from COPD and it is getting worse as time passes. However, this inhaler is working the best out of them all for easing my pains and combivent is easier to use than any other inhalers that I have used in the past. I am suffering from COPD for a little more than 3 years now and since then I am struggling with it daily. As I said, it is getting worse unfortunately, but I am trying to deal with this problem the best way I can. However, it is extremely hard. I guess that’s because I am currently having no insurance and I am unemployed. All of this thanks to the COPD. It has literally forced me to go and seek for a doctor and ask him to get the medications that I really need. But, it doesn’t really matter what would happen, I would use this medicine anytime, anywhere. I love this inhaler above all the other inhalers out there! I am living in Illinois and I am 49 years old female that is using this combivent for about 2 years and a half or so. By the way, the side effects from this inhaler are very small – not even worth to mention them! I am using it about 4 times per day and what I really like about it is that I don’t have the shaking hands thing. I have had it with most inhalers I have been on in the past! Thank you.

I was diagnosed with COPD long time ago and then, later I was told that I’m having bronchi muscle spasm that is a result of the COPD. Currently, I’m on this inhaler and I am on it for approximately 4 or 5 years now. from all other inhalers I have been on I would rather choose combivent in the first place. For me this is the best inhaler out of them all, it works the best for me, it is easier to use than any other inhaler that I’ve been on in the past and I don’t have side effects. I would give it the max rating. Until I found this medication my COPD has been a real struggle. Obviously it all was getting worse and worse as the time passed. Other medications either helped me insignificantly either gave me serious side effects. But this one is just good, it especially helps me in coughing up the mucus from my lungs and helps me to breathe as if I am young again. I have heard about a side effect that many were complaining on: shaking hands. I was afraid that this might affect me too but it doesn’t as I continue using it but there’s no sign of shaking hands. Usually I’m using it for about 4 times per day and it gives me great relief and comfort. I guess that I’m lucky that I’ve found this medication.

I used other inhalers and they don’t work for me, some of them can work but I have side effects, luckily not this one. This drug works very well for me, no side effects is noticed and I can breathe normally. I use it for a little more than one year and overall everything seems fine. Anyway, it has a problem (I guess as any other thing do) – it runs out too quickly, much before it should. It is said that it’s having 200 doses per one inhaler, however it has less than 150 and I’m sure about that. 200 doses should be enough for a month but I’m running out of it a week before the month’s end. Every 4 months I have to pay for another inhaler since insurance will pay just for one refill a month. I have to pay more than I should and I’m frustrated about that. Anyway, this problem has all inhalers I’ve used. I guess all they want to fool people around. It is not a problem that this inhaler in particular has, it’s something they all have.
