Description of the active ingredient

Benemid (probenecid) is an antigout agent. Probenecid inhibits the reabsorption of uric acid in the renal tubules. In addition, probenecid can delay the excretion of other drugs and increase their concentration in plasma. Thus, probenecid may be used as an adjunct to antibiotic therapy.

Indications for use of the Benemid (probenecid)

Hyperuricemia in patients with chronic gout, hyperuricemia associated with the use of diuretics.

The drug is also used as an adjunct to penicillin and some cephalosporins (except cephaloridine).

Benemid (probenecid) dosage

Chronic gout treatment: the initial dose is 250 mg 2 times a day for a month. The dose can be increased to 500 mg 2 times a day after one week of treatment. If necessary, the dose can be increased by 500 mg every month. The maximum dose is 2 g per day.

Probenecid doses may be increased in patients receiving diuretics or pyrazinamide (they increase the concentration of uric acid in the blood).

Combined therapy with penicillin and cephalosporins: probenecid dose in adults is 500 mg 4 times a day. Probenecid dose in children aged 2 years and older is 25 mg/kg followed by an increase to 40 mg/kg. The interval between doses is 6 hours. Children weighing more than 50 kg should take the same doses recommended for use in adults.

Treatment of gonorrhea: probenecid may be used at a dose of 1 g 30 minutes prior to parenteral administration of the antibacterial drug.

Benemid (probenecid) adverse reactions

Digestive system: nausea, vomiting, anorexia, rarely - necrosis of the liver.

CNS: headache, dizziness.

Musculoskeletal system: gout exacerbation.

Urinary system: frequent urination, rarely - nephrotic syndrome.

Allergic reactions: fever, pruritus, urticaria, Stevens-Johnson syndrome, rarely - anaphylactic shock.

Hematopoiesis: anemia, hemolytic anemia (often associated with glucose-6-phosphate dehydrogenase deficiency), rarely - aplastic anemia, leukopenia.

Dermatological reactions: dermatitis, alopecia, flushing.

Benemid (probenecid) contraindications

The drug is contraindicated in acute gout, kidney stone disease, porphyria, circulatory problems, pregnancy, children under 2 years of age, secondary hyperuricemia caused by tumor process or chemotherapeutic agents, hypersensitivity to probenecid.

Benemid is contraindicated in children younger than 2 years.

Pregnancy and lactation

Benemid (probenecid) should be used with caution during pregnancy and lactation and only when the expected benefit of therapy to the mother outweighs the potential risk to the fetus.

Violations of renal function

Caution should be used in patients with chronic renal failure. Probenecid can be inefficient in patients with chronic renal failure.

Benemid (probenecid) special instructions

Benemid (probenecid) is not used in case of secondary hyperuricemia caused by tumor process (probenecid may increase the risk of nephropathy).

Caution should be used in patients with peptic ulcer disease and chronic renal failure. Probenecid can be inefficient in chronic renal failure.

In some cases probenecid may cause acute gout attack, kidney stones, renal colic with hematuria and hemolytic anemia.

Probenecid overdose may cause CNS excitation, convulsions and may result in death due to respiratory failure.

Probenecid should not be used with ketorolac and salicylates. Persons taking probenecid should lower doses of thiopental (for induction of anesthesia).

Probenecid may increase the concentration of the following drugs: acyclovir, captopril, aminosalicylates, lorazepam, paracetamol, rifampicin, indomethacin, meclofenamate, naproxen, methotrexate.

Probenecid reduces urinary concentration of nitrofurantoin and penicillamine.
