Feldene (piroxicam) is a non-steroidal anti-inflammatory drug with analgesic, antipyretic and inflammatory action. The drug effectively reduces inflammation and pain. Feldene capsules are used for the symptomatic treatment of diseases of the musculoskeletal system: osteoarthritis, rheumatoid arthritis, spondylitis, gout, tenosynovitis, bursitis, neuralgia, myalgia, ankylosing spondylitis, back pain, inflammation of the soft tissues, post-traumatic pain. It is sometimes used to reduce the pain in primary dysmenorrhea.
How to recognize Feldene (piroxicam) contraindications
Absolute contraindications: recurring polyps in the nasal cavity and paranasal sinuses, asthma and sensitivity to acetylsalicylic acid or other NSAIDs, uncompensated heart failure, postoperative period after CABG surgery, erosive and ulcerative disorders of the gastric mucosa, gastrointestinal bleeding, inflammatory bowel disease (ulcerative colitis, Crohn's disease), severe and chronic renal insufficiency, III trimester of pregnancy, lactation (breastfeeding), pediatric patients up to 18 years of age, hypersensitivity to piroxicam and other components of the drug, hypersensitivity to aspirin and other NSAIDs.
Use with caution in patients with coronary heart disease, cerebrovascular disease, congestive heart failure, dyslipidemia, hyperlipidemia, diabetes mellitus, peripheral vascular disease, gastrointestinal disorders, Helicobacter pylori infection. Care should be taken in case of concomitant treatment with anticoagulants (warfarin), antiplatelet agents (clopidogrel, acetylsalicylic acid), oral corticosteroids (prednisone), selective serotonin reuptake inhibitors (sertraline, fluoxetine, paroxetine, citalopram).
How to take Feldene (piroxicam)?
Piroxicam should be prescribed only by the doctor who has experience of treating inflammatory or degenerative rheumatic diseases. When used simultaneously with other drugs based on piroxicam, the total dose should not exceed 0.04 g per day.
Feldene capsules should be taken with food. Duration of the course should be determined by the attending physician.
The recommended dosing regimen depending on the disease: Diseases of the musculoskeletal system and joints (osteoarthritis, rheumatoid arthritis, spondylitis): 0.01-0.02 g per day (at once or in 2 doses). If necessary, the daily dose can be slightly increased to 0.03 g or reduced to 0.01 g. Acute diseases of the musculoskeletal system: the first 2 days - 0.04 g a day (at once or in 2 doses), then 0.02 g per day for 1-2 weeks. Acute gout attack: 0.04 g per day for 3-5 days. Dysmenorrhea (first symptoms): the first 2 days - 0.04 g a day (at once or in 2 doses), then 0.02 g per day for the next 1-3 days. Post-traumatic pain: initial dose - 0.02 g once per day. If necessary, increase the dose to 0.04 g (single or divided doses). Do not exceed the recommended daily dose: this may lead to unwanted side effects. The maximum daily dose for adults is 0.04g.
How to recognize Feldene (piroxicam) side effects
Digestive system: nausea, vomiting, dry mouth, stomatitis, epigastric pain, loss of appetite, diarrhea or constipation, flatulence, erosive and ulcerative lesions or bleeding, liver failure. Urinary system: renal failure, interstitial nephritis, nephrotic syndrome, nephrosis, proteinuria, hyperkalemia, papillary necrosis, glomerulonephritis. Central nervous system: somnolence, headache, dizziness, tinnitus; less often - insomnia, depression, irritability, paresthesia, confusion, hallucinations. Hematopoietic system: leukopenia, eosinophilia, anemia, bleeding, thrombocytopenia, thrombocytopenic purpura. Cardiovascular system: hypertension or hypotension, symptoms of heart failure; rarely - shortness of breath, palpitations. Allergic reactions: skin rash, dermatitis, pruritus, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, anaphylactic shock, bronchospasm, vasculitis, photosensitivity, serum sickness, angioedema. Other reactions: hypoglycemia, blurred vision, swelling of feet and legs, hyperhidrosis, weight gain or weight loss. Digestive tract disorders are more likely to occur. Symptoms of overdose: nausea, vomiting, abdominal pain, blurred vision, drowsiness, coma.
Feldene (piroxicam) special instructions
The drug is used only in the lowest effective dose (for the prevention of unwanted side effects). Prolonged use of piroxicam (daily dose of more than 0.03 g) may increase the risk of side effects.
Elderly patients, patients with functional gastrointestinal disorders, bronchial asthma and COPD should monitor the function of liver and kidneys.
