Requip general information
Requip (ropinirole) is the original drug for the treatment of Parkinson's disease both in the early stages of the disease and at later stages (in combination with levodopa preparations). The active substance provides control over the symptoms of the disease both day and night. Ropinirole reduces the severity of hypodynamia, rigidity and tremor, which are symptoms of parkinsonism.
Requip film-coated tablets are used for the treatment of Parkinson's disease both in the early stages of the disease (as monotherapy) and in later stages (in combination with preparations of levodopa).
The drug allows delay levodopa preparations in case of monotherapy in the early stages. In addition, when used in combination with levodopa in the late stages of the disease, Requip increases the effectiveness of levodopa and helps reduce the daily dose of levodopa.
The drug can be taken once a day. Requip helps reduce the severity of clinical manifestations of Parkinson's disease at night and normalize sleep parameters.
Requip indications for use
Parkinson's disease: - Monotherapy of early stages of the disease in patients who need dopaminergic therapy (to delay the administration of levodopa). - As a combination therapy in patients receiving levodopa preparations (in order to increase the effectiveness of levodopa, as well as to reduce the daily dose of levodopa).
Special patient groups
Elderly patients
The clearance of ropinirole is reduced after ingestion by approximately 15% in elderly patients 65 years of age or older (compared with younger patients). There is no need of dose adjustments in this category of patients.
Patients with impaired renal function
The pharmacokinetic parameters do not change in patients with impaired renal function of mild and moderate degree.
Requip contraindications
- Hypersensitivity to ropinirole or any of the components of the drug. - Pregnancy and lactation. - Dysfunction of the liver. - Severe renal dysfunction. - Rare hereditary diseases: lactose intolerance, lactase deficiency, impaired absorption of glucose or galactose. - Children under 18 years of age. - Acute psychosis.
Ropinirole should be administered with caution to patients with severe cardiovascular insufficiency.
Ropinirole can be prescribed to patients with a psychotic disorder only if the expected benefit exceeds the potential risk.
Requip dosage and administration
Adults
Requip tablets should be taken once a day at the same time, regardless of the meals. Take whole, do not break or chew the tablets.
It is recommended to reduce the dose if the patient experiences drowsiness at any stage of dose selection. It is necessary to reduce the dose of the drug with the development of other unwanted reactions.
Monotherapy
The recommended starting dose is 2 mg once a day for one week. The dose can be increased by 2 mg at intervals of not less than 1 week (up to 8 mg per day).
Maintenance dose
If the therapeutic effect is not sufficiently pronounced, you can continue increasing the daily dose of the drug by 4 mg at intervals of 1-2 weeks (until the desired therapeutic effect is achieved).
The dose can be changed depending on the therapeutic effect and increased to a maximum dose of 24 mg once a day.
Combination therapy
When used in combination with levodopa preparations, the dose of levodopa can be gradually reduced (depending on the clinical effect).
What are the possible side effects?
Undesirable reactions are described below, including individual cases.
CNS: hallucinations, confusion, drowsiness, dyskinesia, vertigo, dizziness.
Cardiovascular system: orthostatic hypotension, low blood pressure.
Gastrointestinal tract: very often - nausea, pain in the abdomen, vomiting, constipation.
Immune system: very rarely - hypersensitivity reactions, including hives, angioedema, rash and itching.
Interaction with other drugs
Certain antipsychotics and other dopamine antagonists (such as sulpiride or metoclopramide) may decrease the effectiveness of ropinirole.
Ropinirole does not interact with other drugs used to treat Parkinson's disease.
In patients with Parkinson's disease, who simultaneously took digoxin, there was no interaction of digoxin with ropinirole, which would require correction of doses.
There is no information on the possible interaction of ropinirole and alcohol. However, patients should be warned about the need to refrain from drinking alcohol during ropinirole treatment.
