Guidance for Eldepryl (selegiline) administration

Selegiline is used as a monotherapy in the early stages of the disease or as adjuvant treatment to levodopa therapy. In both cases, the starting dose is 5 mg, which is taken in the morning. The dose can be increased to 10 mg per day (in the morning or in two divided doses). The dose of levodopa should be reduced in case of adverse reactions caused by levodopa therapy.

What are the possible side effects of Eldepryl (selegiline) therapy?

Selegiline is well tolerated as monotherapy. If you start treatment with selegiline, the dose of levodopa can be reduced by an average of 30%.

Mental disorders: insomnia. Central nervous system: dry mouth, involuntary movements (dyskinesia), vertigo, headache. Vascular disorders: hypotension. Violations of the gastrointestinal-tract: nausea.

Eldepryl in combination with non-selective monoamine oxidase inhibitors may cause severe hypertension. Simultaneous use of moclobemide and selegiline may potentiate the hypertensive effect of products containing tyramine (such as fermented foods and beverages, cheese, smoked sausage, liver, salted fish, beans and peas, sauerkraut and products containing yeast).

Avoid using selegiline in combination with pethidine. Tramadol may also interact with selegiline.

Selegiline should be used with caution in combination with dopamine and fluoxetine (high risk of ataxia, tremor, hyperthermia, hyper- and hypotension, cramps, heart palpitations, dizziness and mental health problems (agitation, confusion and hallucinations), delirium and coma. Such reactions may occur in patients using selegiline together with sertraline, paroxetine, venlafaxine and fluvoxamine. It is recommended to avoid using selegiline and the above mentioned drugs.

Care should be taken when using selegiline with selective serotonin reuptake inhibitors (SSRIs) and combined oral contraceptive pills (combination of ethinyl estradiol with progestogen or ethinyl estradiol and levonorgestrel).

Be particularly careful when prescribing selegiline to patients with gastric ulcer or duodenal ulcer, labile hypertension, cardiac arrhythmias, severe angina pectoris, severe liver or kidney disease or psychosis.

High doses of selegiline are associated with the risk of high blood pressure (in patients consuming foods rich in tyramine).

Selegiline is not recommended for pregnant or lactating women.
