Pharmacological action of Plavix
Plavix (clopidogrel) is indicated for the prevention of atherothrombotic complications. This drug helps prevent platelet and thrombus formation. Clopidogrel also causes other reactions that suppress platelet aggregation.
Medical use of Plavix
Plavix is used to treat the following diseases:
- Prevention of atherothrombotic events in patients with ischemic stroke (from 7 days to 6 months after an ischemic stroke);
- Prevention of atherothrombotic events in patients with peripheral arterial disease;
- Prevention of atherothrombotic events in patients with myocardial infarction (up to 35 days after myocardial infarction);

Plavix dosage and administration
The dose for adults is 75 mg per day, regardless of the meal. Patients younger than 18 years of age should take other drugs.
Acute coronary syndrome: the starting dose is 300 mg at once followed by a dose of 75 mg per day in combination with acetylsalicylic acid (from 75 to 325 mg per day). The optimal duration of therapy may be up to 1 year.
Prevention of ischemia in patients after ischemic stroke, myocardial infarction: 75 mg per day in the first days up to 35 days (in case of myocardial infarction) and up to 6 months (in case of ischemic stroke).
Side effects of Plavix
Gastrointestinal tract: abdominal pain, diarrhea, nausea, flatulence, gastritis, constipation, vomiting, peptic ulcer, pancreatitis, hepatitis, colitis.
Central nervous system: dizziness, headache, paresthesia, confusion, hallucination (very rarely).
Hematopoietic system: leukopenia, severe thrombocytopenia, thrombocytopenic purpura, agranulocytosis, granulocytopenia, anemia and pancytopenia/aplastic anemia (very rarely).
Skin: itching, rash, erythema multiforme (bullous rash), urticaria, angioneurotic edema.
Hypersensitivity reactions: anaphylactoid reactions (very rarely).
Respiratory system: bronchospasm (very rarely).
Musculoskeletal system: arthritis, joint pain (very rarely).
Cardiovascular system: low blood pressure, vasculitis (rarely).

How to use Plavix with other drugs
Do not use Plavix in combination with warfarin (high risk of bleeding).
Clopidogrel should be used with caution in combination with heparin.
The combination of Plavix with naproxen increases the likelihood of gastrointestinal bleeding. Nonsteroidal anti-inflammatory drugs should be used with caution in combination with Plavix.
There were no clinically significant interactions between clopidogrel and nifedipine or atenolol.
Antacids do not affect the absorption of clopidogrel.
Do not use Plavix in patients with acute ischemic stroke (Plavix can be used not earlier than 7 days after the ischemic stroke).
Just like other antithrombotic agents, Plavix should be used with caution in patients with a high risk of bleeding due to injury and during surgical interventions. Plavix is administered with caution to patients who take nonsteroidal anti-inflammatory drugs, aspirin, heparin, thrombolytics. Severe cases of bleeding may occur in patients who take aspirin and heparin together with Plavix.
Treatment with clopidogrel should be cancelled a week before surgery.
Use with caution in case of high risk of bleeding from the gastrointestinal tract. Patients must inform the doctor about any unusual symptoms that accompany Plavix therapy.
