What do you need to know about Pletal?

Pletal (cilostazol) is an antiplatelet agent that reduces platelet aggregation. According to experimental data, cilostazol inhibits smooth muscle cell growth. Moreover, it has a vasodilatory effect.

Cilostazol is excreted primarily in the urine. Residual amounts are excreted in the feces. The pharmacokinetics of cilostazol and its metabolites did not depend on the age or sex of patients.

Cilostazol is used in combination with aspirin after stent implantation. It is also effective in patients with intermittent claudication who do not have signs of peripheral tissue necrosis (peripheral arterial disease).

Currently, the drug is also used for the prevention of stroke recurrence, thrombosis and restenosis after reconstructive vascular surgery, as well as for the treatment of post-stroke syndrome.

Pletal main side effects

Cilostazol is relatively safe.

The most common side effects may include headache, transient diarrhea, edema, palpitations and dizziness.

Digestive system and metabolism: often - edema (swelling of the face or peripheral edema), diarrhea, nausea, vomiting, dyspepsia, flatulence, abdominal pain; rarely - gastritis, hyperglycemia, diabetes mellitus, anorexia.

Blood and lymphatic system: often - hematoma; rarely - anemia, thrombocytosis, tendency to bleeding, thrombocytopenia, granulocytopenia, agranulocytosis, leukopenia, pancytopenia, aplastic anemia.

Immune system: rarely - allergic reactions.

Mental disorders: rarely - anxiety.

Nervous system: very often - headache; often - dizziness; rarely - insomnia, abnormal dreams; isolated cases - paresis, hypoesthesia.

Cardiovascular system: often - palpitations, tachycardia, angina, arrhythmia; rarely - myocardial infarction, atrial fibrillation, congestive heart failure, supraventricular tachycardia, ventricular tachycardia, nasal bleeding, gastrointestinal bleeding, orthostatic hypotension; isolated cases - flushing, hypertension, hypotension, cerebral hemorrhage, pulmonary hemorrhage, muscle hemorrhage, bleeding in the respiratory tract, subcutaneous hemorrhage.

Respiratory tract: often - rhinitis, pharyngitis; rarely - dyspnea, pneumonia, cough, interstitial pneumonia.

Hepatobiliary system: rarely - hepatitis, abnormal liver function, jaundice.

Skin and subcutaneous tissue: often - rash, itching; rarely - eczema, Stevens-Johnson syndrome, toxic epidermal necrolysis, urticaria.

Kidneys and urinary tract: rarely - renal failure, impaired renal function; isolated cases - hematuria, pollakiuria.

General disorders: often - chest pain, asthenia; rarely - myalgia, conjunctivitis, tinnitus; isolated cases - hyperthermia.

The number of cases of heart and peripheral edema is higher when using Pletal simultaneously with other vasodilators that can cause reflex tachycardia (for example, dihydropyridine calcium channel blockers).

Pletal main contraindications

This drug is contraindicated in patients with hypersensitivity to cilostazol or any component of Pletal, congestive heart failure, severe renal insufficiency, moderate or severe hepatic impairment, tendency to bleed (such as a stomach ulcer or duodenal ulcer in the acute stage, hemorrhagic stroke, proliferative diabetic retinopathy, poorly controlled hypertension).

Pletal should not be given to patients with ventricular tachycardia, ventricular fibrillation or cardiac ectopy. It is contraindicated during pregnancy. Pletal therapy is not recommended during lactation.

Cautions: The drug is not recommended for children. It should be used with caution in patients with atrial or ventricular ectopy, atrial fibrillation or atrial flutter.

Pletal administration and dosage

The initial dose is of 100 mg 2 times a day. Take on an empty stomach, within 30 minutes or 2 hours before or after lunch or dinner. Food may increase the maximum concentration of the active substance, as well as increase the risk of adverse reactions.

The duration of treatment is from 16 to 24 weeks. The first improvements occur within 4-12 weeks.

Pletal interactions with other drugs and alcohol

Pletal should not be used together with some antibiotics (erythromycin, clarithromycin), antifungal medicines (ketoconazole, itraconazole), calcium channel blockers, benzodiazepines.

Storage conditions
